Important information for any family on the waiver wait list ……

A summary of the information listed below is that families on the wavier wait list need to (1) MAKE SURE they have completed the DDP (Developmental Determination Profile – this can only be done when the student is six years of age or older…some families have put it off thinking they will just do it once the student is targeted for the waiver, but now it is imperative that they schedule it as soon as possible) AND  they (2) call one time a year (845-1646) to make sure BDDS has the families current contact information.

Even if you don’t totally understand what all this is saying, I would encourage families who are  waiting on the wavier list to call BDDS (Bureau of Developmental Disabilities – 845.1646) and at least say, “I know there have been some new requirements for staying on the wait list, I want to do whatever I need to do at this time to ensure my child’s names stays on the wait list.”  It is important that the parent take note of the day/time the call is made and write down the name of the person they talk to (at BDDS. Also, document what the person told them to do (keep this documentation in your file).  This way if the name does somehow disappear from the list, they have documentation that they called.  It is also important to make sure to follow through with whatever it is they need to do to keep the child’s name on the list.
